Consent form for individuals aged 5-14:

Under Oregon law, minors 15 years of age and older may consent to medical treatment, including vaccinations, when provided by a physician, physician assistant, naturopath, nurse practitioner, dentist or optometrist, or other professionals operating under the license of these providers. This consent form is intended to remove vaccine access barriers for individuals 5-14 years old by allowing them to get vaccinated without parental or guardian accompaniment.

If an individual aged 5-14 is not accompanied by their parent or legal guardian, they must have a signed copy of this consent form (Español) or they will not be eligible to receive the vaccine.


Appointment Scheduling

Update: Effective November 9th, all Lane County Public Health clinics will be by appointment ONLY and our scheduling tool will be live by the morning of Friday, November 5th, 2021.

Process to schedule an appointment (first and second dose) with Lane County for a COVID-19 Vaccination:

  1.  Click the "Schedule a Vaccine Appointment" button.
  2. Select whether you are receiving a first or second dose, complete the screening question and contact information.
  3. Clinics may be available on multiple days in multiple locations.  Please select the clinic closest to you.  We ask that you reserve clinic appointments in rural areas for residents of those areas.

Appointment confirmation codes

When you schedule an appointment at a Lane County vaccine event you will receive a specific date, time and location for your appointment along with a confirmation code.  Please bring this code with you to your scheduled appointment.

The confirmation code makes finding you on the appointment schedule easier at check in, but if you don't have it we'll still be able to look you up by your name if you have an appointment.

Please come to the clinic site, date and time that you scheduled for and staff will look you up by name.

Vaccine Record Request (or Lost Card)

Your immunization was recorded in the state ALERT immunization record system. 

If you need a copy of your vaccine information the quickest and easiest way is to complete the form below and email to [email protected] or print and mail to:

Lane County Public Health is limited in our ability to provide you with a copy of your COVID-19 immunization records at this time.  The above resources will provide you with a record more quickly than we are currently able to.



  • Second doses of Pfizer are due 21 or more days after your first does.
  • Second doses of Moderna are due 28 or more days after your first does. 
  •  Johnson & Johnson (Janssen) is a single dose vaccine.
  • Lane County Public Health will not provide the second dose any earlier than the first date you are eligible for the second dose.
  • The second dose needs to be the same brand as your first dose.

To schedule an appointment for a Covid-19 vaccine AFTER HAVING AN UNUSUAL REACTION TO THE FIRST DOSE OF PFIZER OR MODERNA, please call our call center or email [email protected]

In the subject of the email write: Unusual Reaction to Dose 1

In the body of the email write:

  • Name

  • Contact Information

  • Date of 1st Dose

  • Brand of 1st Dose

  • Requested brand of 2nd dose

  • Description of reaction to first dose

We will gather contact information and call people back to schedule appointments for second doses with our local paramedics.


People who began their vaccination series outside the United States with a vaccine not authorized for use in the U.S. should complete the series with a single dose of Johnson and Johnson vaccine.

o  Sputnik (Gam-COVID-vac) – 1 dose of Johnson and     Johnson vaccine at least 21 days after the previous dose.

o  CoronaVac (Sinovac) - 1 dose of Johnson and Johnson     vaccine at least 28 days after the previous dose.

o  AstraZeneca (Covishield) - 1 dose of Johnson and Johnson     vaccine at least 28 days after the previous dose.

• People who are not fully vaccinated with one of the above vaccines may opt to restart a vaccine series with 1 dose of Johnson and Johnson vaccine or 2 doses of either Pfizer or Moderna vaccine.


As of August 12, 2021 the FDA have authorized third-dose COVID-19 vaccines for Moderately to Severely Immunocompromised People. 

Per CDC guidelines this includes people who have:

  • Been receiving active cancer treatment for tumors or cancers of the blood
  • Received an organ transplant and are taking medicine to suppress the immune system
  • Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
  • Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
  • Advanced or untreated HIV infection
  • Active treatment with high-dose corticosteroids* (i.e., ≥20mg prednisone or equivalent per day) or other drugs that may suppress your immune response.†

* ≥20mg prednisone or equivalent per day
† Alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer chemotherapeutic agents classified as severely immunosuppressive, TNF blockers, and other biologic agents that are immunosuppressive or immunomodulatory



Booster dose appointments are released weekly on Fridays. If there are no appointments currently available, please check back.


Update 12/09/21:

 The CDC strongly encourages adolescents ages 16 and 17 to get their booster if they are at least 6 months post their initial Pfizer vaccination series.

At this time, only the Pfizer-BioNTech COVID-19 vaccine is authorized and recommended for adolescents aged 16 and 17.

All people 18 years of age and older who:

            -   Completed a two-dose series of an mRNA vaccine (Pfizer or Moderna) at least 6 months ago

           - Received a Johnson and Johnson vaccine at least 2 months ago

Are eligible to receive a booster dose of either the Johnson and Johnson, Moderna or Pfizer vaccine.

Bring a mask to wear and anything else that will help make you comfortable during your time at the clinic (water, snack). 
We welcome service animals however we ask that you leave non- service animals at home. Typically individuals spend 30-45 minutes total on site depending on the wait at entry. 

You do not need insurance and there is no charge of any kind for the vaccine. 
We will not ask about immigration status. The COVID- 19 vaccine is free regardless of immigration or insurance status.
