Walk-ins are now welcome at Lane County Public Health vaccine clinics. Appointments are still encouraged.

All clinics listed are providing Pfizer brand vaccine (ages 12+) and Johnson & Johnson unless otherwise noted.  If you need a Moderna boost dose please utilize our vaccinating partners  (list of partners is below or call 541-682-1380 for assistance).

The clinics listed above have walk-in availability.  Additional clinic sites, dates and times may be available for scheduled appointments prior to being opened for walk-ins. Consent form for individuals aged 12, 13 or 14:

Under Oregon law, minors 15 years of age and older may consent to medical treatment, including vaccinations, when provided by a physician, physician assistant, naturopath, nurse practitioner, dentist or optometrist, or other professionals operating under the license of these providers. This consent form is intended to remove vaccine access barriers for individuals 12-14 years old by allowing them to get vaccinated without parental or guardian accompaniment.

If an individual aged 12, 13 or 14 is not accompanied by their parent or legal guardian, they must have a signed copy of this consent form (Español) or they will not be eligible to receive the vaccine.


Appointment Scheduling

Walk-ins are welcome at all Lane County Public Health Clinics. 

Scheduling an appointment does help ensure a minimal wait time and quick process as those with appointments will be prioritized.  We aim for individuals to complete the vaccinate and observation process in 30 minutes or less. 

Process to schedule an appointment (first and second dose) with Lane County for a COVID-19 Vaccination:

  1.  Click the "Schedule a Vaccine Appointment" button.
  2. Select whether you are receiving a first or second dose, complete the screening question and contact information.
  3. Clinics may be available on multiple days in multiple locations.  Please select the clinic closest to you.  We ask that you reserve clinic appointments in rural areas for residents of those areas.

Appointment confirmation codes

When you schedule an appointment at a Lane County vaccine event you will receive a specific date, time and location for your appointment along with a confirmation code.  Please bring this code with you to your scheduled appointment.

The confirmation code makes finding you on the appointment schedule easier at check in, but if you don't have it we'll still be able to look you up by your name if you have an appointment.

Please come to the clinic site, date and time that you scheduled for and staff will look you up by name.

Cancel or Reschedule an Appointment

It is no longer necessary to cancel your COVID-19 vaccine appointment if you are unable to make it to a scheduled appointment.  All Lane County Public Health COVID-19 Vaccine clinics are accepting walk-ins and we use these walk-in appointment to continue efficiently using vaccine supplies and adjust for any cancelled appointments.  

If you need to reschedule please use the scheduling form to make your new appointment or attend a clinic as a walk-in (dates above).  You do not need to cancel with Lane County prior to rescheduling.

Vaccine Record Request (or Lost Card)

Your immunization was recorded in the state ALERT immunization record system. 

If you need a copy of your vaccine information the quickest and easiest way is to complete the form below and email to alertiis@state.or.us or print and mail to:

COVID-19 vaccinations for homebound individuals and those in need of transportation assistance.

There is a COVID-19 vaccination program for homebound individuals.  If you are a homebound individual please call or email Lane County (corona-info@lanecountyor.gov or 541-682-1380) and provide a minimum of your address and phone number.  Lane County works with the state who will contact you and schedule a time to come to your home and provide your vaccination. 

Lane County can assist with transportation to a vaccine clinic.  To access this assistance: schedule an appointment and check the box on the scheduling form that says "No, I need help arranging transportation to my appointment" (bottom of the name & demographics page).  Complete the scheduling.  Our team will reach out to you to determine what assistance is needed and help coordinate low or no-cost transportation.  You can also call or email us and we can connect you with our transportation assistance team: corona-info@lanecountyor.gov or 541-682-1380.

If you have a history of anaphylaxis to anything other than an ingredient of the J&J Covid-19 vaccine you can receive the Janssen (J&J) Covid-19 vaccine at McKenzie Willamette Medical Center. Please contact Tory Ketterling at Vketterling@qhcus.com or 541-726-4641 to schedule your appointment. No walk-ins.

Lane County Public Health is working with local providers on access to the vaccine for individuals with a history of anaphylaxis to injectables.  Please call 541-682-1380 for assistance or email corona-info@lanecountyor.gov

When calling to book an appointment you will need to confirm that you have a history of anaphylaxis (severe allergic reaction) to an injectable medication or therapy, but that you do NOT have an allergy to any component of the vaccine.

SCHEDULING YOUR SECOND DOSE

At this time you will need to check the week that you are due. That is when the site repopulates with the upcoming weekend’s clinics.  You can also attend one of our clinics as a walk-in but make sure you are due and that the clinic has the correct brand.  We will not provide the boost dose any earlier than the first date you are eligible for the boost (21 days for Pfizer, 28 days for Moderna).  Most mass vaccination events are providing Pfizer.  If you need a Moderna boost we recommend scheduling with one of our vaccinating partners (listed above).  Please call 541-682-1380 if you need assistance scheduling a Moderna boost.

Autzen Stadium east parking lot.  From Martin Luther King Jr. Blvd. turn in at Entry 3 located at the traffic light at Kinsrow avenue.

Scheduling an appointment does help ensure a minimal wait time and quick process as those with appointments will be prioritized.  We aim for individuals to complete the vaccinate and observation process in 30 minutes or less. 

The clinic is primarily drive through (see below for walk up) and we ask that you remain in your vehicle.  Although you can bring individuals who are not being vaccinated  we do ask that you keep the number of individuals in your vehicle to a minimum.  Please dress appropriately to allow you to easily exposure your shoulder/upper arm.  

Those who use other transportation options (bus, walking, biking etc.) can walk up to the clinic.  The clinic is outdoors in tents and vaccine administration may take place in an area with minimal privacy. Please dress appropriately to allow you to easily exposure your shoulder/upper arm while maintaining your comfort throughout the process.

Bring a mask to wear when interacting with staff and anything else that will help make you comfortable during your time at the clinic (water, snack, blanket).  We welcome service animals however we ask that you leave non-service animals at home. Typically individuals spend 30-60 minutes total on site depending on the wait at entry.  To reduce your total time on site please schedule an appointment.

You do not need insurance and there is no charge of any kind for the clinic.  We will not ask about immigration status. The COVID-19 vaccine is free regardless of immigration or insurance status.

PeaceHealth Riverbend Annex

Florence Events Center

Florence Events Center (715 Quince St, Florence, OR 97439)

Please arrive on-time for your appointment, but no more than 15 minutes early. If you arrive more than 15 minutes before your scheduled appointment time you will need to wait in your vehicle to allow us to maintain appropriate physical distancing inside the building and near the entrances. You will not be permitted to check-in until approximately 15 minutes before your appointment time.

Parking at the Florence Events Center
When you arrive, please park in either the main parking lot on the east side of the FEC or the overflow parking lot at the south side of the FEC. There will be volunteers on site to direct traffic to those with appointments and assist with traffic flow around the FEC.

Entering the Florence Events Center
During your appointment window, please enter the FEC through the main entrance (east side of the facility). Please note, this will be an “entry only” point during the vaccine clinic. When you are done, you will be directed to exit through alternate doors on the south side of the FEC.

Restrooms
During the vaccine clinic, restrooms in the FEC will be closed in order to accommodate the space needed for the flow of the clinic and to maintain physical distancing. The City will have portable restrooms and hand wash stations outside the facility available for those with scheduled appointments.

Prepare
Vaccine administration will take place in a large open room with minimal privacy. Please dress appropriately to allow you to easily exposure your shoulder/upper arm.  We welcome service animals however we ask that you leave non-service animals at home
