Medical Chaperones

Starting July 1, 2023, the Oregon Medical Board requires all health care providers to offer patients a medical chaperone to be present in the exam room during a sensitive exam. Sensitive visits include any examinations, treatments, or procedure of the genitals, rectum, or breasts.

At Community Health Centers of Lane County, we want you to be comfortable during all exams and procedures. We have trained chaperones to make sure sensitive exams and procedures are done in a safe, comfortable and professional way.

A medical chaperone is trained to know what to expect during an exam and is there to ensure patient and provider safety. The chaperone cannot be a personal friend or relative of the patient.

You may choose to decline to have a chaperone present. Please speak with your health care provider if you have questions about a sensitive exam or the use of a medical chaperone.

Family Practice & Internal Medicine

Our medical providers deliver a range of acute, chronic and preventive medical care services. They diagnose and treat illnesses, as well as provide routine checkups, health-risk assessments, immunizations and screening tests.  Our Internal Medicine physicians work with patients in the management of their chronic illnesses.

Pediatrics

The Pediatric program provides services for children and youth, from birth until they are 21years old.  Our pediatric providers are available to see children for well baby and well child exams, immunizations, sports physicals, acute and chronic conditions. Reproductive Health

Community Health Centers of Lane County participates in the Oregon Reproductive Health Program making reproductive health exams and contraceptive supplies available for free or low-cost.  We can send a prescription for birth control to your pharmacy and we also stock a variety of options in our clinics, such as multiple types of birth control pills, long acting reversible contraceptives like IUDs and implants, the patch, the ring, and condoms. Well-woman exams, cancer screening and prevention, STI testing and treatment, and pregnancy testing are also offered at all of our locations.  Prevention is a cornerstone of our services and our providers and nurses are trained to offer patient-centered information on preventing unplanned pregnancy and sexually transmitted infections.


There have been recent changes to the Title X program at the national level. Title X is a source of federal funding for reproductive health care to people with low incomes. These changes do not impact our services or your ability to receive reproductive health care. Our clinic is no longer receiving Title X funds to pay for reproductive health services. Clients seeking care at our clinics will continue to receive the same high-quality, client-centered reproductive health services that have always been available.

The Whole Person - Integrated Behavioral Health

At Community Health Centers we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness but also health, education and behavioral health services.

The Integrated Behavioral Health (IBH) program is available to patients as a part of overall good healthcare. The purpose of this service is to offer assistance when stress, worry, chronic health conditions, or emotional concerns about physical or other life problems are interfering with someone's daily life. The IBH provider works as a member of the primary care team. This team approach considers physical, behavioral, and emotional aspects of our patient's health.

Dovetail Program

What is the Dovetail Program?

Dovetail is a referral-based Community Health Worker (CHW) program that is integrated into Community Health Centers of Lane County. The goals of the Dovetail Program are to strengthen patients’ abilities to address life challenges through short-term, goal-oriented support; to connect patients to community resources, help them navigate systems, and overcome barriers to access services.

Our short-term program generally focuses on addressing 1 – 2 specific goals with participants, and we meet with our participants by appointment in clinic or at community locations. CHWs work collaboratively with patients to address social barriers to health, support them in staying on track with their identified goals, and build their skills to navigate systems independently. Success depends on the patient’s active participation and commitment to the process.

If you have been referred to Dovetail by your primary care provider, you will work directly with a CHW for up to ten 1-hour visits. Your Dovetail CHW will support you in setting goals, building skills, and accessing available community resources to reach those goals. Currently, our program does not have any special access to resources beyond what is available in the community. Nor are we able to assist in crises or time-sensitive situations.

CHWs are currently integrated into the following clinics: Brookside, Charnelton, Delta Oaks, RiverStone, South Lane, and West 11th.

What is a Community Health Worker and what can they do?

A Community Health Worker (CHW) is a trusted member of the community who empowers individuals through education and connection to social services and community resources. CHWs work in health systems with the goal to improve both individual and community well-being.

CHWs are uniquely qualified to do this important work due to their abilities to:

  • Build strong, trusting relationships.
  • Bring their lived experiences and deep understanding of their communities and cultures into their work.
  • Empower Through Education and Advocacy
  • Provide culturally appropriate education, coaching, and support to build patients’ skills and confidence in managing their goals.
  • Act as advocates to ensure equitable access to social services and community resources.
  • Serve as a bridge between patients, providers, and agencies to improve understanding, access, and follow-through. Assist patients in understanding forms and processes to help patients gain independence.

Low-Cost Medications Programs

Community Health Centers of Lane County offers a variety of assistance and medication discounts programs for our patient.  These special programs include allowing patients fill their prescription at select local pharmacies such as Safeway Pharmacy.  The medication discount program offers free home delivery for those that select a mail order option.  We encourage all of our patients to take advantage of the services offered by our partner pharmacies.  Our healthcare team will help you identify the best option for you.

Dental Prevention Program

The Dental Prevention Program serves children in Head Start, Early Head Start,  Preschool Promise, Relief Nursery, ECCares, Migrant Summer Educational Program, elementary and middle schools throughout Lane County. Onsite prevention clinics with portable equipment are set up onsite at all participating schools. These clinics offer dental hygiene assessments, teeth cleaning, fluoride varnish treatments, sealants, Silver Diamine Fluoride and education. Homecare packets include toothbrushes, toothpaste, and floss. Services are provided twice a year.
 

The goals and objectives of the Dental Prevention Program are:

  • To establish dental homes
  • To achieve Healthy People 2030 oral health goals by reducing the proportion of children with untreated decay, in primary and permanent teeth, increase the number of children with sealants in permanent molars, apply fluoride varnish for preventing dental caries in children and adolescents, and reduce the overall incidence of decay.
  • To reach the American Academy of Pediatrics and American Academy of Pediatric Dentistry goal of having first dental check up by eruption of first tooth or by one year of age.

Why provide dental prevention services to children?

  • 34 million school hours are lost each year due to dental disease, CDC< May15,2024. Healthy children learn better.
  • “On any given day, more than 6,000 children in Oregon are in school and suffering from dental pain or infection.” Oregon Department of Human Services, Oregon Smile Survey 2017.
  • Centers for Disease Control and Prevention recommends school-based and school-linked sealant programs to reduce prevalence of dental disease. Cavities are a bacterial infection that may be prevented by sealants. Sealants seal out the germs that cause cavities in molars where 85% of dental disease occurs.
  • Dental caries remains the most prevalent chronic disease in children, even though it is largely preventable,  according to National Institute of Dental and Craniofacial Research, November 2022.

Why is cavity prevention important?

  • Dental infection affects heart, lungs, and general health.
  • Dental disease is costly to restore, and may result in tooth loss.
  • Cavities may affect speech development, learning and the ability to eat nourishing food. Establishing healthy dental care habits leads to a lifetime of better health.

Integrated Oral Health

What is Integrated Oral Health?

Integrated Oral Health (IOH) is a dental prevention program focused on the vital connection between oral health and overall systemic health. We provide holistic care, recognizing that good oral health is essential for overall well-being, and that general health can also impact oral health. Our goal is to educate, motivate, and empower our community to achieve optimal health through good oral care.


Our program serves children of all ages, adults with diabetes, pregnant women, and individuals with emergent dental needs referred by their primary care provider. We offer services at the Delta Oaks Clinic in Eugene, the WIC offices in Eugene, and the RiverStone Clinic in Springfield. Current services include dental hygiene assessments, pediatric teeth cleanings, fluoride varnish treatments, sealants, Silver Diamine Fluoride, dental care coordination, referrals, and oral hygiene instruction. We look forward to expanding our services to reach more individuals in the future.
