General Information
To enter a Domestic Partnership in Oregon, a Declaration of Domestic Partnership must be registered with a County Clerk. This declaration is a legal form and must be completed in black or dark blue ink. The form cannot be altered.
OHA Domestic Partnership Registration Form
The form must be printed single-sided on two separate pages. All sections must be completed unless otherwise indicated in the instructions. All signatures must be acknowledged by a Notary Public.
Definitions
Domestic Partnership
A Domestic Partnership is a civil contract entered between two individuals who are at least 18 years old, are legally capable, and where at least one partner is an Oregon resident.
Partner
An individual who is joined in a domestic partnership.
Criteria for Entering a Domestic Partnership
A domestic partnership is prohibited and void:
- When either party already has a declared partner, wife, or husband living at the time of the domestic partnership
- When the parties are first cousins or nearer of kin (whole or half-blood), as computed under civil-law rules
- When either party is legally incapable of entering a contract due to age or insufficient understanding
- When consent is obtained through force or fraud. In such cases, the partnership is void from the time it is declared void by a court with jurisdiction
Note: If the parties are first cousins by adoption only, the partnership is not prohibited or void.
Submitting Your Declaration
Once completed, signed, and notarized, submit the form with the $60.00 fee (in person you may pay with cash, check, or card) by mail or deliver to:

Upon filing, the declaration will be signed by the County Clerk or Deputy County Clerk and entered into the Domestic Partnership Registry.
A copy of the completed form, along with a Certificate of Registered Domestic Partnership, will be returned to the partners in person or by mail.
