Oregon Judicial Department Forms

 

The Oregon Judicial Department has a select number of forms made officially available for legal matters. Local courts such as Lane County Circuit Court has forms created for some situations not addressed by the state-wide forms. There are not always official forms for every legal issue, but the form you need may already be available. It is recommended to double-check the OJD Forms Center and the Lane County Circuit Court Forms to see if there is an official form for the situation.


 

The OJD Website also has some search functionality. After searching your search term, select “Forms” below the search bar. This can be helpful as it will also show any local court pages.


 

It is important to always ensure on your documents that the correct county is in the title of the form or pleading. If you are filing in Lane County, ensure it has “Lane” in the title.

 

Drafting Your Own Court Documents

 

As there are not official forms for every circumstance, sometimes you need to draft your own form. Legal documents can only be drafted by you and your lawyer, library staff are not a substitution for a lawyer. Upon request, we offer legal resources and sample forms to help bridge the gap of justice information. The Pleading Paper Template below is designed to be used for personal drafting circumstances.



Sample forms are generally NOT fill-in-the-blank, but rather informational guides and examples to help you draft your document. Patrons are expected to review and assess the content in the examples to draft their own documents, along with relevant information and law specific to their case.

The Oregon Judicial Department has standards and guidelines as to what a pleading, or court document, should look like. If you need to draft your own pleading, the pleading templates are editable documents meant to comply with UTCR 2.010 but should be double-checked for accuracy. Pleading Paper Templates are NOT a form, they only are the formatting of a court document.


The Law Library has Microsoft Word available on the Public Computers. If you do not have Microsoft Word at home, there is also a version that is compatible with Google Docs below.

Can I E-File through OJD?

You can file a limited selection of forms online via the Oregon Judicial Department Guide & File System.

 


“Oregon circuit courts provide online forms (sometimes called iForms) for self-represented court users to electronically complete forms and file or respond to cases. Guide & File is an online program that works on any computer or mobile device.”

Guide & File is available for protection orders, divorce, separation, or custody cases, changing a family judgment, developing a parenting plan, application for a court-appointed lawyer (criminal cases only), residential evictions, small claims complaints, name or legal sex change, motion to seal your eviction, marijuana, or criminal record (expungement), satisfaction of a money award, and firearms surrender declaration.

 


The library offers public computers if you need computer or internet access.

Can I Purchase Any Forms?

Stevens-Ness is an Oregon Forms Dealer for purchasable Oregon and Washington forms ranging from $9.95 to $45.95, depending on the circumstance. They have forms related to Landlord/Tenant law, Simple Estate, Deeds and Trusts, and more.

Why Can't I Find the Form I'm Looking For?

There is an access to justice gap throughout the United States, and that does not stop in Lane County. Legal Services Corporation, a national funder of civil legal aid, reported that "low-income Americans did not receive any or enough help for 92% of their civil legal problems."

 

County Law Libraries try to bridge the gap by providing alternative legal resources and sample forms. There is not a fill-in-the-blank style form for every circumstance, so you may need to draft your own form.

Where Else Can I Find Sample Forms?

The Law Librarian and Library Staff can point you to resources with sample forms, such as our databases like Westlaw, LexisNexis, and BarBooks. Most of our databases do not have remote access, so it is best to come into the library to utilize these resources. If you are looking for remote resources, please click here.
